Aujourd'hui n'a pas été une journée trop compliquée, la plupart de mes patients n'ayant pas beaucoup de nouveautés, mais je suis quand même un peu découragé d'avoir encore 2 semaines de gériatrie devant moi... C'est un domaine où le psycho-social prend pratiquement deux fois la place de la science médicale brute. Il est constamment question de physiothérapie, d'évaluations en neuropsy et de centres de réadaptation, et une fois tout l'aspect médical réglé il est courant que les personnes âgées restent sur l'unité pendant des semaines en attente d'une relocalisation. Moi qui préfère les trucs ponctuels et les investigations de problèmes aigus, ça fait maintenant deux semaines que je fais le suivi de certains patients pour qui l'essentiel du travail médical est fini depuis longtemps. :(

Au moins, j'avoue qu'il y a quelques bons côtés à ce stage: l'absence de résidents et la permissivité des patrons fait que je peux m'occuper de mes patients presque comme bon me semble. Je peux décider à quelle heure je commence ma journée, dans quel ordre je visite les patients, combien de temps je passe avec chacun, quels tests je demande, et même quels changements je fais à leur médication avec un niveau d'interférence minimal des patrons. Il n'y a personne dans ce service pour harceler, têter, chialer et envahir l'espace des externes comme le font typiquement les résidents obsessionnels d'une unité d'enseignement. Après un stage de chirurgie où il y avait tout le temps des résidents et où je ne pouvais presque jamais organiser ma journée à ma manière, ce changement d'ambiance est vraiment le bienvenu !

En gériatrie, je procède de la façon la plus confortable pour moi: un bon café en arrivant à l'hôpital, ensuite un blitz de travail "industriel" sans la moindre pause à partir de 9 heures, puis normalement j'arrive à midi débarrassé de toutes mes tâches et il ne me reste pour l'après-midi que de menus détails, du temps libre, et des circonstances propices pour partir très tôt de l'hôpital.

Mais pour les trois prochains jours, le plan de match est encore plus réjouissant: il n'y a tout simplement pas de plan, et je prévois ne rien faire du tout. Congé ! :P

This Sunday night, RTE will broadcast 'Whistleblower' the first of a 2-part drama series based on the true events outlined in the enquiry into irregular obstetric practices at Our Lady of Lourdes Hospital, Drogheda. The Lourdes Hospital Inquiry Report by Judge Maureen Harding Clark was published two and a half years ago and highlighted the plight of 129 women whose lives were blighted forever by unnecessary caesarian-hysterectomies carried out by obstetrician, Dr. Michael Neary. The investigation described the number of peripartum hysterectomies performed at Drogheda as "truly shocking". It's probably the worst case of systems failure in Irish medical history. It took the tremendous bravery of a midwife to blow the whistle in 1998, to put a stop to the wrong doing.

Dr Michael Neary has never explained the failure in his duty of care to his patients nor has he ever apologised. What he did was tantamount to abuse over a period of 30 years, he ruined people's lives. How did this happen? How could 129 women lose their wombs unnecessarily? The vast majority of these women were young, 25 of them were first-time mothers. The report praised the management of the hospital and the health board for dealing promptly and appropriately in setting up the investigation to take-on the consultant. However, opinions still vary on the reasons why Dr. Neary did what he did and nobody, except Dr. Neary, knows the real answer.

I talked to my Doctor about what was going on and she has decided to take me off my current medication for two days and then start a half dose of a new medication for a week. I will go back in one week to see how things are going. If things seem well, then she will up the dose to the normal amount. She also gave me a refferral to a counselour that she would like me to see at least twice. We will see if this happens. I'm not sure what my insurance will cover on that and if it will really do any good. Hopefully I'll come up on an even keel soon.

Dua minggu di Bagian Anak dan aku lagi-lagi melihat kerasnya dunia. Anak-anak yang lahir dengan kelainan kongenital: ASD kek, VSD kek, ada yang kena double; anak yang kekurangan gizi: mpe kurus...banget; ada yang kena penyakit yang juarang...banget kasusnya (dan entah ini keberuntungan atau kebuntungan) kok ya aku ketemu sama pasien itu: kardiomiopati restriktif, hemofilia B pada anak perempuan 11 tahun (baru tahu gw klo cewek bisa manifest hemofilia juga, selama ini kirain cuma bisa jadi carier. Hm....). Dari semua itu... kok kayanya pengetahuan gw masih dikit banget ya? Hh...
