<p>The largest health insurer in Ohio is following the lead of Medicare and will stop paying hospitals for costs of certain preventable mistakes and infections.</p>
<p>Anthem Blue Cross and Blue Shield announced Wednesday that as of Oct. 1 it will no longer pay for errors such as surgeries performed on the wrong body part. The insurer listed 11 categories in which it won't cover extra costs of treatment, including pressure sores, urinary tract infections from catheters left in too long and hospital-acquired fractures.</p>
<p>Anthem's parent company, WellPoint Inc., initiated the change in Blue Cross and Blue Shield plans it operates in 14 states, said Kim Ashley, a spokeswoman for Anthem Blue Cross Ohio. Anthem covers about 1 million people in Northeast Ohio and more than 3 million statewide, not including those in Medicaid managed care plans, she said.</p>
<p>The policy follows the plan the government announced last year for Medicare.</p>
<p><a name="more"></a>Hospital infections alone are blamed for about 100,000 deaths a year in the United States, and $27 billion in costs, according to Consumers Union.</p>
<p>Tiffany Himmelreich of the Ohio Hospital Association said she was not sure if any other Ohio insurers had adopted the policy. Hospitals are concerned about "gray areas" -- conditions that in some cases may not be preventable, she said.</p>
<p>"There is still a lot up for debate," Himmelreich said. "How do we pay or not pay for something when it's not crystal clear what type of event it is. Is it preventable or not."</p>
<p>Hospitals are not allowed to bill patients for costs Anthem declines to pay under the policy, Ashley said.</p>

<p class="content">“This good news means Medical Mutual’s HMO and POS plans have maintained their Excellent Accreditation Status under the Managed Care Organization (MCO) accreditation review,” said Sauer. “It also indicates Medical Mutual’s PPO plans received Excellent Accreditation Status the first time they were evaluated under MCO accreditation review.”</p>
<p class="content">The Excellent Accreditation Status was awarded after rigorous evaluation by NCQA of all aspects of Medical Mutual’s plans, including preventive health services, member satisfaction, physician credentialing and quality improvement.</p>
<p class="content">The review included both on and off-site evaluations conducted by a team of physicians and managed care experts. A national oversight committee of physicians then analyzed the team’s findings and assigned Medical Mutual’s Excellent Status based on the Company’s performance compared to NCQA standards.</p>

<p>It's a place I had always wanted to visit--northern Saskatchewan--and fish its abundant waters for pike, lake trout and Arctic grayling. I had been in the north before, both as a biologist and an angler, and I was hooked. The country is so different and wild from what we know in the south, and there is so much territory to explore. So, when I had a chance to visit a fly-in fishing lodge in northern Saskatchewan, I took it.</p>
<p>The lodge was <a href="http://www.gwnlodges.com/Minor_Bay/Minor_Bay.html" target="_blank">Minor Bay</a> and is located on Wollaston Lake. We flew from Saskatoon to Points North Landing near the lake. The trip from Saskatoon revealed a large landscape of abundant water--lakes and streams as far as the eye can see. Once at Points North, we were transported to the lodge in one of my favorite bush airplanes, the deHaviland Beaver.</p>

<p>Once at the lodge, we were quickly put into boats to go look for lake trout. The ice had only recently left this end (south) of the lake, so the trout were still relatively close to the surface. We caught a few but nothing of size. All were released. The policy at Minor Bay is to release all fish, except for a few smaller ones for shore lunch. This ensures there will always be large fish to catch in a climate that only provides three or so months of growth.</p>

<p>In the following days we fished for monster pike both on the lake and at a fly-out location. The guides ensured we found some great fish and we had a great time fighting some beautiful fish--all returned to fight again. My fishing partner, <a href="http://www.stammphoto.com" target="_blank">Doug Stamm</a>, caught a nice trophy pike on a fly.</p>

<p>As well, we took a little time to fish for Arctic grayling on the Wheeler River. Once we figured out what we were doing, we caught some very pretty fish.</p>
<p>All in all it was a great trip with lots of variety and good people helping us out. I would recommend this lodge if you are looking for a place that knows how to treat anglers while ensuring the conservation of the resource.</p>

Muchos expertos a nivel mundias los consideran una medida que realizada  de forma indiscriminada no es existosa. Los descastes de esócidos que apoyan los ecologistas conservadores que desean la desaparición de esta especie, cuentan con la mala fama de eliminar los grandes ejemplares. La falta de grandes lucios ha mostrado en muchas aguas producir el efecto opuesto al deseado: una explosión demográfica de pequeños Esox lucius, que ejercen una predación mucho mas importante sobre pequeñas truchas y ciprínidos. Después de tantos años intentando exterminar al lucio, los esócidos sobreviven, con poblaciones saludables en numerosas masas de agua.</p>

<p>La pesca eléctrica resulta inútil cuando se realiza en tablas profundas. Para muchos, los descastes son un intento en vano de frenar la colonización del Rey Esox. Además, resulta triste ver tanto lucio muerto, cuando existen masas de agua donde, las puestas llegan a perderse por los continuos trasvases y desembalses en época de freza. A pesar de ello, el lucio constituye una de especies deportivas de agua dulce mas importantes y cada día mas apreciadas por los pescadores. Mientras tanto, crecen las asociaciones de pescadores como <a href="http://www.szczupak.org/" target="_blank">Save The Pike</a>, a favor de una legislación razonable y coherente con el panorama actual, que establezca unos cupos de capturas, así como unas medidas mínimas para esta especie.</p>

<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:14pt;" lang="EN-GB"><span style="font-family:Times New Roman;">Earlier I stopped halfway across the bridge to watch the water flowing and generally listen to the silence.<span>  </span>A splashing on the other side of the bridge drew my attention and swimming gaily along came a hen Mallard and three ducklings.<span>  </span>They appeared to have not a care in the world. (The river is a popular place for countless ducks to breed.)<span>  </span>BUT, in the not so deep depths a killer lurked; a pike about 2 feet long ‘hovered’ static in the current and watched.<span>  </span>With scarcely any effort it rocketed to the surface, there was a flurry of water and mother duck and two ducklings continued on their merry way.</span></span></p>
<p class="MsoNormal" style="text-align:justify;margin:0;"><span style="font-size:14pt;" lang="EN-GB"><span style="font-family:Times New Roman;">Within one minute there was nary a ripple nor sign that one life had been snuffed to sustain another.</span></span></p>
